Randomized trial of citrulline administration in patients hospitalized in intensive care for COVID-19 acute respiratory distress syndrome
Respiratory involvement from SARS-CoV-2 leads to ARDS and significant immunosuppression (lymphopenia), exposing patients to prolonged ventilation and late mortality related to the acquisition of nosocomial infections. The lymphopenia characteristic of severe forms of ARDS secondary to SARS-CoV-2 infection may be linked to the expansion of MDSCs and arginine depletion in lymphocytes. Furthermore, severe forms of COVID-19 pneumonia are marked by persistent ARDS with the acquisition of nosocomial infections, as well as by prolonged lymphocyte dysfunction associated with the emergence of MDSCs. Correcting hypoargininemia could be an effective therapy to rapidly correct lymphocyte dysfunction, reduce the acquisition of ICU-acquired infections, and accelerate respiratory clearance of the virus.
