• Mutational order shapes cell state and chromatin accessibility during early colorectal cancer evolution

Using CRISPR mediated gene editing in patient-derived human colon organoids, we investigate how mutational order shapes cell state and downstream phenotype. We find that loss of FBXW7 on a wild-type background produces minimal phenotypic or transcriptomic change but induces a subtle shift towards a foetal-like, regenerative stem cell identity. Strikingly, the order of mutation acquisition produces markedly different outcomes. 'FA' organoids (i.e. FBXW7 loss preceding APC mutation) retain a near-normal morphology and transcriptional profile. In contrast, 'AF' organoids adopt a proliferative stem cell state and a more transformed phenotype, despite identical final genotype. These differences were associated with distinct chromatin accessibility landscapes, and reflect a cell state-dependent constraint on the transcriptional consequences of subsequent mutations.

• Research on constructing a genome analysis database for malignant tumors

Germline pathogenic variants (GPVs) in BRCA1/2 are established cancer predisposition factors; however, their biological and clinical significance in gastric cancer (GC) remain unclear. In particular, the directionality and evolutionary timing of locus-specific loss of heterozygosity (LOH) and their relationships with homologous recombination deficiency (HRD) have not been systematically examined in BRCA1/2-associated GC. We conducted integrative genomic analyses of four GCs from BRCA1/2 GPV carriers using tumour-normal tissue paired whole-exome sequencing and allele-specific copy number profiling. LOH directionality was inferred by integrating the variant allele fraction, tumour purity, and locus-specific copy number. The relative timing of LOH events during tumour evolution was estimated, and HRD was evaluated using genome instability scores and mutational signature analyses.

• Malignancy of breast cancer cells is increased in a luminal-hybrid state defined by progenitor markers CD146 and keratin 14

We first analyzed a patient-derived xenograft (PDX)-derived breast cancer cell line by dividing cell populations based on CD146-expression by FACS and found that this marker defines a cellular state with increased expression of a selection of luminal hybrid markers, including KRT14. We then knocked out CD146 or KRT14 in another patient-derived xenograft (PDX)-derived breast cancer cell line to investigate further the role of these markers in the hybrid state. For all these populations, we studied their gene expression by bulk RNA sequencing.

• Transcriptomic characterization of breast cancer cells in a luminal-hybrid state

Background: We have previously identified a subset of early human breast epithelial progenitors that, despite their luminal affiliation, are enriched for basal markers, including CD146 and keratin 14 (K14). This simultaneous presence of both lineages thus defines a luminal-hybrid state. As luminal progenitors are generally proposed as a cell of origin of breast carcinoma, we examined how cells within this luminal hybrid state relate to breast cancer malignancy. Findings: We first analyzed two patient-derived xenograft (PDX)-derived breast cancer cell lines by dividing cell populations based on CD146-expression by FACS and found that this marker defines a cellular state with increased expression of a selection of luminal hybrid markers, including KRT14. When subjecting the sorted cells to in vitro transwell filter analysis, CD146HI/K14HI luminal hybrid state cells were less invasive. Knocking out either CD146- or K14-expression by CRISPR/Cas9 on a CD146/K14-positive background did not affect the in vivo tumorigenic potential, nor the differentiation state. However, when subjecting the respective sorted populations to a soft agar colony formation assay or as in vivo xenografts, CD146HI/K14HI cells exhibited more growth and metastasis, suggesting that the luminal hybrid state contributes to malignant properties. Conclusions: Our findings demonstrate that multiple components of the luminal hybrid state provide distinct properties to tumor malignancy. The fact that breast cancer can reflect a significant part of the original breast gland differentiation hierarchy may add weight to the argument that the search for novel strategies for treating basal-like breast cancer should include manipulation of cellular differentiation states.

• NGS sequencing data of archival FFPE tumor tissue

The aim of the study was to identify mutations in key ovarian cancer genes (BRCA1, BRCA2,TP53, PTEN, ATM, ATR, NF1) in archival fresh-frozen paraffin embedded (FFPE) tumor tissue from platinum-resistant ovarian cancer patients enrolled in the phase I/II GANNET53 clinical trial. From the enrolled 133 patients, archival FFPE tissue was available and DNA was extracted thereof. The DNA samples passing quality control (n=118) were NGS sequenced using the SureSeq™ Ovarian Cancer Panel and the NGS Library Preparation kit (Oxford Gene Technology), which covers all exons of 7 key ovarian cancer genes for single nucleotide variants (SNV) and indels, and the Illumina MiSeq platform. For the paired-end runs, one Read 1 (R1) and one Read 2 (R2) FASTQ file were created for each sample. FASTQ files were compressed and created with the extension .fastq.gz.

• Tissue architecture and immune niches govern ctDNA release in colorectal cancer

Circulating tumor DNA (ctDNA) is central to liquid biopsy-based cancer detection, yet its release into the bloodstream varies widely and remains poorly understood. To define the tissue-level determinants of ctDNA shedding in colorectal cancer (CRC), we integrated tumor-informed plasma sequencing with detailed histopathology, immunophenotyping, spatial transcriptomics, and in situ mutation detection in resectable stages (I–III). ctDNA detectability increased with tumor burden, and high ctDNA shedders exhibited a distinct architectural and microenvironmental phenotype characterized by expanded necrotic pseudolumina, frequent epithelial barrier disruption, and dense myeloid infiltration. Spatial profiling revealed stress-associated malignant programs and a myeloid-rich immune-luminal niche. In situ sequencing confirmed plasma-detected mutations within pseudoluminal debris, identifying these structures as focal reservoirs of shed DNA. These findings provide a mechanistic framework linking tissue architecture, immune remodelling, and spatially organized cell death to ctDNA release with implications for refining liquid biopsy applications.

• FGFR2b Prevalence in Biliary Tract Cancers

Patients with intrahepatic cholangiocarcinoma (iCCA) often have limited effective treatment options after first-line therapy. Actionable driver mutations are present in only a small subset of patients with iCCA; therefore, therapeutic strategies such as antibody–drug conjugates (ADCs) that do not exclusively rely on presence of specific oncogenic mutations may be particularly attractive. However, there is a critical lack of knowledge regarding prevalent and actionable cell-surface targets in iCCA. We applied a novel isoform-resolved proteogenomic discovery framework for profiling cell surface targets and identified FGFR2b, a splicing isoform of FGFR2, as a therapeutic target in iCCA that is highly-expressed, tumor-enriched, localized to the membrane, and associated with prognosis. FGFR2b is the dominant isoform in iCCA, and it was present in all patients with FGFR2 fusions. These findings provide a strong rationale for further research and therapeutic development of FGFR2b-targeted biologics, including therapeutic antibodies and ADCs, in iCCA.

• DAC for Early detection of ovarian cancer using cell-free DNA fragmentomes and protein biomarkers

Ovarian cancer is a leading cause of death from gynecological malignancies worldwide. No effective screening methods exist resulting in tumor detection at advanced stages where treatment is less effective. In this study, we used whole-genome cell-free DNA (cfDNA) fragmentome and protein biomarker (CA-125 and HE4) analyses to evaluate 591 women, mostly post-menopausal, from the European Union or the United States with ovarian cancer, benign adnexal masses, or without ovarian lesions. Using a machine learning model that incorporated multi-analyte fragmentome data and protein measurements, we evaluated the detection of ovarian cancer in two clinical settings, detection of ovarian cancers among a healthy population, and classification of adnexal masses as benign or malignant. The combined approach detected ovarian cancer with specificity >99% and sensitivity of 72%, 69%, 87%, and 100% for stages I–IV, respectively (AUC=0.96, 95% CI: 0.94-0.99), including 90% of high grade serous ovarian cancers. At the same specificity, CA-125 alone detected 34%, 62%, 63%, and 100% of ovarian cancers for stages I–IV (p=0.001, two-sided test of equal proportions). Additionally, our approach differentiated benign masses from ovarian cancers with high accuracy (AUC of 0.88, 95% CI=0.83-0.92). These results were validated in an independent population. These findings show that integrated cfDNA fragmentome and protein analyses detect ovarian cancers with high performance, enabling a new accessible approach for noninvasive ovarian cancer screening and diagnostic evaluation.

• Prefibrotic Bone Marrow Microenvironment is a Hallmark of Clonal Hematopoiesis

Clonal hematopoiesis of indeterminate potential (CHIP) is linked to systemic inflammation and increased malignant risk, yet its impact on the bone marrow (BM) niche remains poorly understood. Through single-cell transcriptomic profiling of freshly isolated BM, we identify fibro-inflammatory rewiring as an early hallmark of CHIP. Beyond a myeloid-biased inflammatory state, CHIP BM exhibited coordinated TNFα activation across hematopoietic progenitor, monocyte, and stromal compartments. Stromal populations displayed early fibrotic remodeling, with mesenchymal and vascular cells upregulating collagen and extracellular matrix programs. Fibroblasts were markedly expanded independent of mutational subtype and acquired cancer-associated fibroblast (CAF)-like programs, rewiring niche interactions through IL-6, TGFβ, and collagen-integrin signaling. Together, these findings establish fibro-inflammatory BM remodeling as an early hallmark of clonal expansion, linking CHIP to prefibrotic stromal remodeling, immune dysregulation, and a niche architecture consistent with increased malignant risk.

• Ex vivo therapy-driven selection of glioblastoma stem-like cells models resistance evolution

Glioblastoma (GBM) remains lethal, creating an urgent need for experimental models of therapy- specific resistance evolution. Within-patient GSC models were developed by parallel ex-vivo selection under temozolomide (TMZ-GSC) or ionizing radiation (IR-GSC), alongside untreated controls (CTRL- GSC). • O6-methylguanine-DNA methyltransferase (MGMT) expression was confirmed as a determinant of primary TMZ resistance and was maintained during GSC evolution. • In MGMT-negative GSCs, primary TMZ sensitivity evolved into stable secondary resistance through mismatch-repair gene inactivation, recapitulating patient findings, or was retained along with a newly identified drug-tolerant phenotype. • Primary radioresistance was commonly observed, while its further evolution was associated with more efficient DNA double-strand-break repair and enrichment of homologous- recombination and checkpoint-control transcriptional programs. • In GSCs emerging after either TMZ or IR, broad transcriptional patient-specific programs and classification-relevant subtypes were largely preserved. • Across treatments, GSC evolution was accompanied by shared genome-maintenance and mitotic chromosome-segregation transcriptional programs and increased RTK expression and activity, collectively promoting recovery after genotoxic stress.

• The Precision Medicine in Liver Cancer across an Asia-pacific NETwork (PLANET) – HCC recurrence cohort

Hepatocellular carcinoma (HCC) is a highly fatal cancer marked by significant intra-tumor heterogeneity (ITH). Given 50% to 70% of HCC patients experience recurrence, understanding the origins and progression of recurrence is crucial. To address this, we assembled a multi-sector, paired HCC recurrence cohort of 24 patients, including comprehensive whole genome sequencing and transcriptome samples. Through this longitudinal cohort, we aimed to trace the origins of recurrence and identify molecular features associated with early recurrence. These insights may improve predictions of early HCC recurrence and inform potential therapeutic strategies.

• A spatiotemporal cancer cell trajectory underlies glioblastoma heterogeneity

Abstract: Cancer cells display highly heterogeneous and plastic states in glioblastoma, an incurable brain tumour. However, how these malignant states arise and whether they follow a tractable cellular trajectory across tumours is poorly understood. Here, we generated a deep single cell and spatial multi-omic atlas of human glioblastoma that pairs transcriptomic, epigenomic and genomic profiling of 12 tumours across multiple regions. We identify that glioblastoma heterogeneity is driven by spatially-patterned transitions of cancer cells from developmental-like states towards those defined by a glial injury response and hypoxia. This cancer cell trajectory regionalizes tumours into distinct tissue niches and manifests in a conserved manner across tumours as well as genetically distinct tumour subclones. Moreover, using a new deep learning framework to jointly map cancer cell states and clones in situ, we show that tumour subclones are finely spatially intermixed through glioblastoma tissue niches. Finally, we show that this cancer cell trajectory is intimately linked to myeloid heterogeneity and unfolds across regionalised myeloid signalling environments. Our findings define a stereotyped trajectory of cancer cells in glioblastoma and unify glioblastoma tumour heterogeneity into a tractable cellular and tissue framework.

• PBMCs Bellini TNBC anti-PD1 monotherapy

Immune checkpoint blockade (ICB) combined with chemotherapy (CT) has improved outcome in high-risk early-stage triple-negative breast cancer (TNBC); yet it is unclear which patients benefit from the addition of ICB beyond chemotherapy alone. We profiled 67 serial tumor and 175 blood samples obtained before and during anti-PD1 treatment across 3 breast cancer trials using single-cell transcriptomic and T cell receptor (TCR) sequencing. Early intratumoral T cell expansion emerged as a consistent hallmark of clinical benefit. This expansion was reflected in increased TCR sharing between tumor and blood after treatment initiation, with shared blood T cells displaying immune-activated programs and their tumor counterparts enriched for expanding clonotypes, suggesting blood-to-tumor trafficking of activated T cells. In TNBC treated with anti-PD1+CT, T cell expansion distinguished an immune-driven response from a chemotherapy-dominant route to pathological complete response (pCR). To enable baseline patient stratification, we derived a 15-gene tumor signature predictive of T cell expansion, which associated with response in independent breast cancer cohorts and with improved survival across ICB-treated cancer types. Our data establish T cell expansion as an early marker of sustained ICB response and could allow stratification of patients likely to benefit from the addition of ICB to chemotherapy.

• G4SCOPE identifies sequence features and epigenetic contexts of chromatin G-quadruplexes

Cancer-associated epigenetic remodeling involves both DNA secondary structures and covalent cytosine modifications, yet predicting chromatin-constrained G-quadruplex (G4) folding remains challenging. Here, we develop G4SCOPE, a multi-task deep learning framework that predicts G4 occupancy and quantitative CUT&Tag signal directly from DNA sequence in isogenic human breast epithelial cells. Trained across wild-type, TP53-deficient, TP53/BRCA1-deficient and TP53/BRCA2-deficient backgrounds, the model integrates convolutional motif learning with transformer-based self-attention to capture local motifs and broader contextual sequence features without auxiliary epigenomic inputs. It distinguishes chromatin-associated G4s from GC-matched loci with in vitro G4-forming potential, captures quantitative variation in local CUT&Tag signal, and performs consistently across the four breast epithelial genotypes. Independent application in K562 cells further supports the portability of the framework to a distinct cellular context. Model-derived G4 representations also separate loci with distinct local 5-methylcytosine and 5-hydroxymethylcytosine profiles. Together, these results show that DNA sequence can predict chromatin G4 occupancy and quantitative CUT&Tag signal, and that the learned representations reflect differences in local cytosine-modification state.
